Anyone can transform their body just with a good diet, but even better when combining with exercise and/or weight training, my advice if going to a gym is to use a gym where there are instructors who will give you an assessment of what your aim is and create a routine and give you advice as to your diet. After that use the internet to learn about nutritional requirements and training. If you look at the headings at the top of the page you will see "Workouts" (different type of workouts) "Video's" (400+ videos of exercises for different bodyparts) "Nutrition" which has articles, recipes, supplements video's and nutrition guides, but anything your not sure of, post your questions up on this forum and you will receive an answer

Yes, it is possible but i would recommend that you should ask someone about some instruction so that you can effectively gain muscles. I think you should have a proper consultation with the Origym Physical Fitness Experts who would guide you each and every aspect of body building.

For weight loss, it was just small things at first. I cut all fast food and junk food (like chips/cookies) out of my diet. I don't eat chips/dip anymore, I literally can't eat fast food anymore without it making me sick. I started drinking water rather than sugary/fruity drinks. When I snacked, I started eating pistachios, raisins, cucumbers, or raw almonds instead of Doritos or popcorn or something like that. After that, I started walking a lot, taking stairs and just mostly not being lazy. Then I started running outside. It was hard at first, building up my endurance again after years of not running regularly.

For toning, I found that yoga did it best for me. It's great for strength-training and even my friends were noticed that my legs and arms had more definition to them than before. It was really a struggle because I had no upper-body strength and a lot of the positions work on both upper and low body strength at the same time so I always felt self-conscious of my arms shaking haha.
